Admission Criteria
To qualify for insurance-paid, in-home services, one must have a medical need(s) that requires skilled medical care and attention.
Conditions requiring skilled care include, but are not limited to:
- Education about your health care needs, medication, and/or treatment
- IV medication therapy
- Surgical or wound dressing changes
- Catheter insertion and related care management
- Joint replacement therapy and rehabilitation
- Functional decline due to progressive illness
- Therapy for a fracture that limits functioning
- Falls risk management and ambulation training (with or without a can or walker)
- Pain management
- Swallowing assessments and programs
- Low vision home adaptation
Your doctor or care coordinator (if you are being discharged from the hospital or skilled rehabilitation facility) can help you best determine if you need home health care. |
