Admission to the Hospice program
is appropriate when:
The patient, of any age, has a diagnosed
limited life expectancy as determined by the
patient’s primary physician.
The patient and family consent to Hospice
Care Services.
The patient has a primary caregiver who is
willing and able to assume or arrange for 24-hour
responsibility for the patient. The program
supports, but does not take over for, family
members.
The patient and family agree to a treatment
plan based on providing care and comfort rather
than cure.
The patient’s primary physician and
Hospice’s Interdisciplinary Group consent
to home Hospice admission.
