Hospice Admission Criteria

Admission to the Hospice program is appropriate when:

• The patient, of any age, has a diagnosed limited life expectancy as determined by the patient’s primary physician.
• The patient and family consent to Hospice Care Services.
• The patient has a primary caregiver who is willing and able to assume or arrange for 24-hour responsibility for the patient. The program supports, but does not take over for, family members.
• The patient and family agree to a treatment plan based on providing care and comfort rather than cure.
• The patient’s primary physician and Hospice’s Interdisciplinary Group consent to home Hospice admission.
