	CHANS medical social workers provide education and assistance in finding resources so you can remain safely in home.

Medical Social Work Services

Medical Social Workers identify and access resources available in the community to help the patient remain safely at home. The Social Work Team also provides support to help the patient and family cope with the stresses and problems caused by illness or injury.

Our social workers can assist the patient with accessing:

• community resources
• housing and basic needs
• nursing or boarding home options
• in-home care options
• financial and legal information and resource referral
• advance directives and living will information
• insurance issues/questions
