What Skilled Care Provides

 
  CHANS nurses provide complete assessments at each home visit. Our nurses conduct health check-ups, and assist patients with organizing medications, in addition to answering patient questions.

Skilled Nursing Services

Registered nurses assess physical, emotional, and environmental needs of the patient and family. They work in conjunction with the patient’s physician and coordinate all home care services. Our registered nurses are experienced home health practitioners with special training and credentials.

Nursing care can encompass:

  • teaching the patient more about his/her illness or condition
  • coordinating treatment resources
  • supporting the patient and family through the transition from hospital to home
  • assessing medication side effects and compliance
  • providing wound and ostomy care
  • administering injections, IV therapy, central line care and/or lab work
  • teaching proper diabetic care
  • helping the patient with pain management techniques
  • instructing the family in patient care with respect to medications, nutrition, treatments, and prevention
  • consulting with the patient’s physician on cardiac rehabilitation home care programs
  • assessing patient and family issues with respect to physical and/or emotional needs of the patient
