- Bereavement Services are
a component of Hospice services, provided by
the CHANS Home Health Care Hospice social work
staff, with the aim of supporting the family
and care givers in their bereavement and grief.
- Certified home
care aides assist as often as daily with personal
care and light housekeeping.
- Hospice Care has contracts with the two local
hospitals (Mid Coast and Parkview) to provide
inpatient services when necessary. Inpatient
stays are short-term with the goal of getting
the client back home as soon as possible.
- Hospice
Care arranges and pays for equipment, medication,
and supplies related to the terminal illness.
- Mental health
nursing services personnel are available to
consult with the patient and family, hospice
team, and physician about the mental health
needs of the patient should signs of depression
or anxiety require specialized attention.
- Under the direction of the
primary physician, registered nurses coordinate
all services and visit the home to do physical
assessment, teaching, management of symptoms,
and pain control. They are available on call
24 hours a day, 7 days a week.
- Our staff recognizes
that offering flavorful and/or comforting food
is symbol of care and concern for the patient
and family.
- CHANS Home Health
Care office hours are 8:00 a.m. to 4:30 p.m.
Eastern Standard Time, 7 days/week. A full range
of after hours services is available to CHANS
patients by calling CHANS Home Health Care/Hospice
at (207) 720-6782, ext. 0.
- Local clergy are contacted
to provide spiritual support and pastoral counseling
at the request of the patient or family. Our
own resident chaplain is available to provide
supportive services to augment visits and care
provided by the patient’s own clergy.
- A licensed social
worker is available to counsel the patient and
family regarding emotional and psychological
issues, or to assist with financial or legal
issues.
- Physical Therapists, occupational
therapists, and speech and language pathologists
are provided when needed. There is also a registered
dietitian available for consultation.
- Through a contract with
Hospice Volunteers of Mid Coast Maine, volunteers
are provided for in-home companionship and respite.
In addition, various bereavement support groups
are staffed by trained and caring volunteers.
All of these services are available to the patient
and family, but not all are needed or utilized
by everyone. |
