What Skilled Care Provides:

CHANS Home Health Care provides patient care based on a model of care called ‘medical case management.’ Case management is a collaborative process designed to facilitate the recommended treatment plan to assure that appropriate medical care is provided and the lines of communication between providers and the patient are clear. All CHANS patients will have a case manager, which can be a nurse or therapist based upon the patient’s need, who will oversee the patient’s treatment plan.
  • Nursing Services: Under the direction of the primary physician, registered nurses coordinate all services provided by CHANS Home Health Care and visit the home to do physical assessment, treatment, teaching, management of symptoms, and pain control. They are available on call 24 hours a day, 7 days a week.

  • Home Care Aide Services: Certified home care aides provide personalized care to assist with hygiene and dressing.

  • Physical Therapy Services: Physical Therapists provide a wide range of therapy services designed to increase mobility and independence, while decreasing pain and symptoms. CHANS therapists follow rehabilitation protocols specific to each of the local orthopedic practice groups and procedures to ensure continuity of care and to increase positive outcomes.

  • Occupational Therapy Services: CHANS Home Health Care occupational therapists provide services designed to increase strength, balance and mobility, and independence in activities of daily living.

  • Speech and Language Therapy Services: Specialized services provided by the speech and language pathologists are designed to educate, treat and assist in achieving optimal results for those patient’s struggling with the after effects of injury, stroke or disease that make speech and swallowing difficult.

  • Social Work Services: A licensed medical social worker is available to assist the patient and family with financial or legal issues, community resources and planning of needs once CHANS Home Health Care services are completed.

  • Mental Health Nursing Services: Mental health nursing provides services, medication management and treatment to patient’s with known psychiatric disorders, as well as those who are struggling with the emotional issues related to their medical diagnosis.

  • On Call Services: CHANS Home Health Care office hours are 8:00 a.m. to 4:30 p.m. Eastern Standard Time, 7 days/week. A full range of after hours services is available to CHANS patients by calling CHANS Home Health Care at (207) 720-6782, ext. 0.

Specialty Programs

  • Asthma Education: A specialty pediatric and adult service provided by a certified asthma educator who will provide in home educational visits designed to assist parents and caregivers in better managing a patient’s asthma.

  • Diabetic Teaching: A subset of our nursing services is a specialized focus on providing teaching about self care, nutrition and medications to assist the patient in more effectively managing his/her diabetes.

  • Maternal and Child Health Services: Our pediatric nurses, physical, occupational and speech therapists provide care and services to children who have medical and/or therapeutic needs and provide their families with education and support to assist children in the recovery process.

  • Infusion Nursing Services: CHANS Home Health Care nurses can provide specialized care for those needing infusion (IV) therapy to treat nutritional and supplemental needs or infection.

  • Wound Care Services: CHANS Home Health Care nurses and wound care specialist are adept at providing wound care services based on physician orders, while monitoring and coordinating with the physician and wound care clinics to achieve the best possible results.

All of these services are available to the patient and family, but not all are needed or utilized by everyone.
